Overview

Resource and advocate for Movement Center program’s newly diagnosed and existing patients. Candidate will work closely with patients and families ensuring all medical center and community resources are available on a continuing basis.

Responsibilities

  • Assesses patient's social services and discharge planning needs and their ability to provide for those needs following hospitalization.
  • Identifies situations indicating possible abuse, dependencies, growth and development problems, and sexual orientation issues and follows up appropriately.
  • Provides support to patients and families coping with illness and hospitalization.
  • Makes recommendations regarding needs of patients and suggestions for providing more effective comprehensive care and social work services.
  • Refers patients to appropriate community agencies for concrete services following discharge.
  • Recognizes possible cases of abuse and refers to appropriate agencies for investigation and follows-up as appropriate.
  • Documents all assessments, referrals and interventions in the patient's medical record.
  • Maintains monthly statistics with regard to patients served and completes monthly report.
  • Receives formal in-service on all CentraState Nursing Care Facilities including Level of Care and admission criteria.

Qualifications

Education:

Bachelor's Social Work Required or BS/BA in Social Work or in the related fields of Guidance and Counseling, Human Services, Marriage and Family Counseling, Psychology, Sociology, and Vocational/Disability Rehabilitation, from an accredited college or university. Internship required.

 

Work Experience:

2 years of Medical Social Work Experience Preferred

Familiarity with chronic illness patients and families, demonstrate ability to utilized community resources.

 

Licenses and Certifications:

Certified Social Worker / New Jersey Preferred

BLS Required

Non-violent Crisis Intervention certification required.  New hires must receive certification within 3 months of hire.  Renewal certifications will be completed every 2 years.

 

Required Knowledge and Skills:

Resource Facilitator has background in social work or related field to assist patients in the Movement Program and families with both medical center and community resources. These resources may include mental health resources, respite care, transportation or financial resources, as an example. The individual will be responsible for support groups both virtual and in person. It is the expectation that the individual will identify opportunities to improve the experience of patients and family at CSMC.

Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team.
